The three failures
The three takeovers below failed in three different ways, and between them they cover most of what walks through our door. Natshoot: the developer who would not release the source code. Hunters Portal: the people who built it were simply gone. GolfPlayed: five years of developers who each added to it and none of whom would own it. Different causes, same position for the business, which is a system you depend on and nobody who can change it.

The one we talked out of it
RoloNext asked for a rewrite. The audit said no.
RoloNext arrived with the decision already made: rewrite all of it. The audit split the system instead. The API and the UI did need rebuilding. The mobile app did not, and a refactor is a fraction of the work, the time, and the cost of a rewrite.
It is the only study here where what we delivered was advice rather than a system, and it is the one that best explains how we work.
